Welcome to Horizon Clinical Group Ltd. These Terms and Conditions apply to the use of our website and the purchase of our private adult ADHD assessment services.

By booking an appointment or purchasing a service from Horizon Clinical Group Ltd, you agree to these Terms and Conditions. Please read them carefully before booking.

1. ABOUT HORIZON CLINICAL GROUP

Horizon Clinical Group Ltd is a private, nurse-led clinical service providing virtual adult ADHD assessment and diagnostic services.

Clinical assessments are undertaken by a Registered Mental Health Nurse registered with the Nursing and Midwifery Council (NMC), working within their professional scope of practice, competence and relevant professional standards.

Horizon Clinical Group is an independent private service and is not part of, or affiliated with, the NHS.

2. ELIGIBILITY AND LOCATION

Our assessment service is available to adults aged 18 years and over who are ordinarily resident in the United Kingdom, subject to our clinical suitability criteria and geographical restrictions.

Professional boundaries and regional restrictions

To maintain appropriate professional boundaries and avoid actual or potential conflicts of interest arising from other professional commitments, Horizon Clinical Group does not accept bookings from individuals who fall within our restricted geographical area.

For the purposes of our current service, we are unable to accept clients whose residential or billing address falls within certain SA postcode area’s.

If a booking is made despite this restriction, Horizon Clinical Group reserves the right to cancel the appointment and issue an appropriate refund in accordance with our Cancellation and Refund Policy.

This restriction is a professional boundary relating to the operation of the service and does not affect access to NHS services or other healthcare providers.

3. NATURE AND SCOPE OF THE SERVICE

Horizon Clinical Group currently provides adult ADHD assessment and diagnostic services only.

Our service does not currently provide:

  • ADHD medication prescribing

  • medication initiation

  • medication titration

  • ongoing medication management

  • pharmaceutical treatment

  • acute or emergency mental-health care

Our assessment process may include a detailed clinical history, developmental history, screening questionnaires, collateral information where available, recognised diagnostic assessment tools and a structured clinical interview.

Where clinically appropriate, the assessment may include consideration of coexisting or alternative conditions and may result in recommendations for further assessment or onward referral.

Assessment methodology

Horizon Clinical Group uses a structured, evidence-informed approach to adult ADHD assessment. The DIVA-5 may form an important component of the assessment process.

No single questionnaire, screening tool or diagnostic interview is used in isolation to determine a diagnosis. Diagnostic conclusions are based on the overall clinical assessment, available information, recognised diagnostic criteria and professional clinical judgement.

4. DIAGNOSTIC OUTCOMES

An ADHD assessment is an independent clinical assessment and does not guarantee a diagnosis of ADHD.

The assessment fee relates to the professional time, clinical expertise, assessment process and report provided. It is not payment for a predetermined diagnostic outcome.

Where the assessment does not support a diagnosis of ADHD, this does not in itself constitute grounds for a refund.

Where appropriate, the assessment may identify an alternative explanation for the difficulties described, recommend further assessment, or suggest onward referral to another healthcare professional or service.

5. REPORTS AND RECOMMENDATIONS

Following completion of the assessment, an appropriate written report will be provided in accordance with the service purchased.

Where clinically appropriate, reports may include practical recommendations relating to functional difficulties and potential workplace or educational support.

A diagnostic report does not guarantee that an employer, educational institution, Access to Work, benefits authority, insurer, GP or other organisation will accept a particular diagnosis or recommendation.

Horizon Clinical Group cannot guarantee eligibility for benefits, including Personal Independence Payment (PIP), Access to Work funding, workplace adjustments or any other form of financial or practical support. Decisions regarding eligibility or reasonable adjustments are made by the relevant organisation or authority.

6. MEDICATION AND FUTURE CARE

Horizon Clinical Group does not currently prescribe, initiate, titrate or manage ADHD medication.

If, following assessment, you wish to explore medication or other treatment options, you will need to arrange this separately with an appropriately qualified prescribing clinician or healthcare provider.

Any future development of additional services by Horizon Clinical Group will be subject to appropriate clinical, professional, regulatory and governance requirements and will not form part of the service currently purchased unless expressly stated otherwise.

7. SUITABILITY FOR ASSESSMENT

An ADHD assessment may not be clinically appropriate for every individual.

Before or during the assessment process, we may determine that further information, additional assessment or onward referral is required before an ADHD diagnostic conclusion can safely or reliably be reached.

Where appropriate, we may recommend that you seek assessment or treatment from another healthcare professional or service.

We reserve the right to decline or discontinue an assessment where it is not clinically appropriate, where sufficient information cannot reasonably be obtained, where there are significant concerns regarding immediate risk, or where the assessment falls outside the scope of the service.

Where an assessment is discontinued or cannot proceed, any refund will be considered in accordance with our Cancellation and Refund Policy and applicable consumer law.

8. CONFIDENTIALITY, SAFEGUARDING AND INFORMATION SHARING

We respect your privacy and confidentiality and handle personal and clinical information in accordance with our Privacy Policy and applicable data-protection requirements.

Information provided during an assessment will normally remain confidential.

There may, however, be circumstances in which information must or may be shared without your consent, including where there is a serious risk of harm, a safeguarding concern, a legal requirement to disclose information, or where disclosure is otherwise permitted or required by law.

Where appropriate and safe to do so, this will be discussed with you.

Horizon Clinical Group does not provide an emergency or crisis service. If you are experiencing an immediate mental-health emergency or are at immediate risk of harm, you should contact emergency services, your GP or the appropriate urgent mental-health service.

9. FEES AND PAYMENT

Assessment fees are displayed on our website and at the point of booking.

Payment is required at the time of booking unless an alternative payment arrangement has been expressly agreed.

Payments may be processed through third-party payment providers, including Stripe and Klarna.

Horizon Clinical Group does not store customers’ full payment card details. Payment information is processed in accordance with the relevant payment provider’s terms and privacy arrangements.

Klarna

If you choose to use Klarna, your payment arrangement and any applicable credit agreement are subject to Klarna’s own terms and eligibility criteria.

Horizon Clinical Group is not responsible for Klarna’s credit assessment, approval decisions, payment arrangements or the operation of its credit services.

10. CANCELLATION AND REFUNDS

Our Cancellation and Refund Policy forms part of these Terms and Conditions.

Where a statutory cancellation right applies, this will be honored in accordance with applicable consumer legislation.

If you expressly request that Horizon Clinical Group begins providing the assessment service before the end of any applicable statutory cancellation period, you will be asked to acknowledge this request at the time of booking.

Where a service has begun at your express request during the cancellation period and you subsequently cancel before the service has been fully performed, any amount payable will be determined in accordance with applicable consumer legislation and will reflect the proportion of the service supplied where applicable.

Where the service has been fully performed during the cancellation period following your express request and the required acknowledgement, your statutory right to cancel may no longer apply.

11. APPOINTMENTS AND RESCHEDULING

Appointments are delivered virtually using the platform specified by Horizon Clinical Group.

Clients are responsible for ensuring that they have:

  • a suitable private environment;

  • a reliable internet connection;

  • appropriate audio and video facilities; and

  • sufficient uninterrupted time to participate in the assessment.

Where a client is unable to attend or participate appropriately, the appointment may need to be rescheduled.

12. EXTERNAL HEALTHCARE PROVIDERS

If you choose to use your assessment report when seeking further care, treatment, medication or prescribing from another healthcare provider, it is your responsibility to establish whether that provider will accept an independent diagnostic assessment or report.

Different healthcare providers, NHS organisations, Health Boards and other services may have their own clinical and administrative requirements.

Horizon Clinical Group cannot guarantee that another provider will accept or act upon our assessment report.

13. PROFESSIONAL INDEMNITY AND CLINICAL RESPONSIBILITY

Clinical services are provided within the scope of the clinician’s professional competence and applicable professional standards.

Horizon Clinical Group maintains appropriate professional arrangements for the services it provides.

Nothing in these Terms and Conditions is intended to exclude or restrict any liability or legal right that cannot lawfully be excluded or restricted.

14. COMPLAINTS

We aim to provide a professional, respectful and high-quality service.

If you have a concern or complaint, please contact us using the details provided below. We will acknowledge and manage complaints in accordance with our complaints procedure.

Where appropriate, you may also have the right to raise concerns with relevant professional or regulatory bodies.

15. PRIVACY AND DATA PROTECTION

Our Privacy Policy explains how Horizon Clinical Group collects, uses, stores and protects personal and clinical information.

We will process personal information in accordance with applicable data-protection legislation.

16. CHANGES TO THE SERVICE

Horizon Clinical Group may review and update its services, policies and procedures from time to time.

Any significant change to the nature or scope of the clinical service will be considered in accordance with applicable professional, legal and regulatory requirements before implementation.

The service you purchase will be the service described at the time of booking.

17. GOVERNING LAW

These Terms and Conditions are governed by the law of England and Wales.

Nothing in these Terms and Conditions affects any statutory rights you may have as a consumer.
